We pride ourselves on coordinating care with highly specialized Physicians and to assist in the recovery in a timely manner. Our vast knowledge and working relationships with a multitude of Specialists gives us an edge. We initiate care by triaging the condition and then set up care with the appropriate Specialist.
We provide onsite case management services which includes a same day appointment summary with a work status attached. There are no administrative fees and we have competitive pricing.

DME/Diagnostics/Medications
Center For Case Management coordinates services such as DME (durable medical equipment), diagnostics including X-rays, CAT Scans and MRI testing and ensures receipt of medications related to the work injury/recovery. Transportation, translation and home health services are also coordinated when needed. These services are given high priority to expedite the recovery process.

We work closely and coordinate with various Physical, Speech, Occupational Therapies to ensure the best care is provided. We strive at maintaining constant contact with the treating therapies for effective communication which results in the best overall care.

At Center For Case Management, we offer professional, reliable case management services to all of our clients. We coordinate care with Specialists that covers a range of injures such as (and not limited to) hand, shoulder, neck, back, head, knee, hip, abdominal, elbow, ankle and foot.

Service You Can Trust
Once a referral has been made we will make contact with the injured worker and employer within 24 hours (if not the same day). A confirmation email will then be sent to the adjuster with appointment information.
